Uprima new oral treatment option to be made available for impotence patients
Abbott Park, Illinois -- Abbott Laboratories announced today that it has received a marketing authorisation from the European Commission for UPRIMA(R) for the treatment of impotence. UPRIMA (apomorphine hydrochloride) is the first centrally acting oral therapy to be approved by the European Commission for the first-line treatment of impotence. Impotence is a medical condition affecting an estimated 30 million men in Europe and more than 150 million men worldwide. UPRIMA is a small tablet that dissolves quickly when placed under the tongue, typically producing an enhanced erectile response in under 10 minutes.
"Every patient is different, and UPRIMA's unique product attributes allow docotrs to provide therapy that meets the needs of individual patients," said William Dempsey, senior vice president, international operations, Abbott Laboratories.
Novel mode of Uprima action
UPRIMA is a dopamine receptor agonist that works in the brain to improve diminished erectile function by enhancing the natural signal to the penis following sexual stimulation, similar to the way men normally have erections. UPRIMA works through the central nervous system, producing a series of events that enhances the ability to achieve and maintain penile erection. It is distinct from other oral therapies, which act by blocking the action of certain enzymes involved in the erectile response.
Because UPRIMA is sublingually administered (under the tongue), it enters the bloodstream quickly, bypassing the gastro-intestinal tract. The rapid absorption of UPRIMA into the bloodstream allows for its fast onset of action, typically under 10 minutes.

Uprima Positive Clinical Trials
UPRIMA has been extensively studied in clinical trials involving more than 5,000 patients, with more than
120,000 doses of UPRIMA administered. UPRIMA was studied in men with varying severity of erectile dysfunction
and included patients with organic, psychogenic or mixed etiology erectile dysfunction. The effectiveness

Could men who suffer from impotence possibly have a drug that's easier to use than Viagra (sildenafil, Pfizer)? Apparently, they might one day. A Prominent advisory committee has recommended the approval of a sublingual product for erectile dysfunction. Called Uprima (apomorphine), the drug is actually an old-time dopamine agonist used off-label for the management of Parkinson's disease (PD). Apomorphine's benefits in erectile dysfunction were initially observed in PD patients who were given subcutaneous injections of the drug for motor fluctuations. Five out of the 15 PD patients, who had previously suffered from sexual dysfunction, experienced frequent erections following administration of apomorphine (2.4 to 5 mg two to six times daily) for an average of 18 months. It is believed that this agent induces penile erections through stimulation of central D2 dopamine receptors.
Taking matters a step further, Tap Pharmaceuticals developed a sublingual dosage form of apomorphine that is rapidly absorbed and is reported to work in 10 minutes. In more recent, 44% of men who received a 2-mg dose of Uprima had success attempting and achieving an erection, compared with 38% of those given placebo. At higher doses, the success rate jumps to 60%, but not without an increase in the drug's main side effect--nausea. Don't forget: Apomorphine is a narcotic derivative. And although the drug's analgesic effects are diminished, many of its narcotic effects are maintained, including induction of emesis and the ability to produce CNS depression and respiratory depression.

Data Demonstrates Long-Term Efficacy if Uprima in Impotence Treatment

New therapy effective in 92 percent of attempts at intercourse over an 18-month period in men who respond to UPRIMA
Rome -- Men with erectile dysfunction (ED) who responded to UPRIMA (apomorphine hydrochloride SL) showed consistently high rates of success over 18 months, according to data presented at the 4th Congress of the European Society for Sexual and Impotence Research (ESSIR). Across the entire study period, an average of 92 percent of attempts resulted in an enhanced erection firm enough for intercourse among patients taking UPRIMA 2 mg. Efficacy was measured as the percentage of attempts resulting in an erection firm enough for intercourse with UPRIMA, according to patient and partner diary assessments. Both patient and partner evaluations confirmed the success rate.
The findings provide important evidence of the long-term efficacy of UPRIMA, the first centrally acting oral therapy to be approved for the first- line treatment of erectile dysfunction.
Speaking at ESSIR, Professor Jeremy Heaton of Queen's University and Kingston General Hospital, Ontario, Canada, said, "There is no indication that patients develop tolerance to UPRIMA with long-term use. These results provide further reassurance that men with ED who respond to UPRIMA can expect consistent results from UPRIMA over an extended period."
Recent data published in the Journal of Urology(2) concluded that "there is a possible tachyphylaxis effect with sildenafil." Tachyphylaxis is a decrease in responsiveness to the drug over time with repeated exposure. The report continues, "Of the patients who were followed for two years, 20 percent had to increase the sildenafil dose to have the same effect, and 17 percent discontinued use due to loss of efficacy." A meta-analysis was performed on the long-term Phase III clinical studies to assess the efficacy of UPRIMA 2 mg in patients who completed the short- term, placebo-controlled, Phase II/III studies and elected to continue into long-term, open-label Phase III studies.
In patients who elected to continue using Uprima 2 mg for impotence treatment, 92 percent of all attempts
with Uprima over this period resulted in an erection firm enough for intercourse. Continued success was observed
in patients who stayed on Uprima 2 mg for longer periods, 97 percent of all attempts resulted in enhanced erections
firm enough for intercourse.
